The information you provide here is used to build the production version of your app and prepare its listings for the Apple App Store and Google Play Store. Please ensure you have completed and submitted App Config One Guide (Staging) before starting this form.

This section covers the text and keywords that will be displayed on your app's public store page. This is your chance to attract and inform potential users.
These are very short, punchy sentences that appear prominently in the app stores. Keep them concise (max 80 characters for Android, 30 for iOS) and focus on the main benefit of your app.
This is the detailed overview of your app. It should be engaging and informative, explaining what the app does and the value it provides. Consider including a strong opening hook and testimonials to build credibility.
Enter the name of the person or entity that owns the exclusive rights to the app, preceded by the year the rights were obtained (e.g., "© 2025 Your Company Inc.").
List several keywords that describe your app's purpose (e.g., "wellbeing, health coaching, sleep, nutrition"). These are crucial for helping users discover your app through search.
App store listings require screenshots that showcase your app in action. We will generate the final, high-resolution screenshots for you based on a test user you set up.
Please take simple screenshots of the key screens from your test participant's account in the staging app and upload them here. We will use these as a direct reference to create the final, professionally formatted screenshots for the app stores. You must provide at least three, and up to six.
This is a specific promotional image required only for the Google Play Store.
This is a branded banner image used in various places on the Google Play Store, sometimes as a cover image for your preview video. Please upload a PNG file that is exactly 1024px wide by 500px high.
Google requires you to declare how your app uses health and medical data. Please review the following sections and select all features that apply to your app.
Select all the general wellness categories your app addresses (e.g., Nutrition and weight management, Sleep management).
Select all the medical categories your app addresses. Be aware that selecting categories here may trigger additional reviews from the Google Play team.
Select Yes if your app is used for formal research studies or clinical trials.
Provide a clear, user-facing description of how your app uses the health data it collects. This text helps both users and store reviewers understand the purpose of the data sync.
Please provide a link to a publicly accessible video (e.g., on YouTube or Vimeo) that visually demonstrates how a user enables health data syncing in the app and how that data is then used. This is often a requirement for passing the store review process.
Please list all the countries where you want your app to be downloadable.
These contact details are for the app store review teams (Apple and Google) to use if they have any questions or issues during their review. This information will not be made public.
